MENTAL HEALTH Partial Hospitalization Program (MH-PHP)

The Partial Hospitalization Program (PHP) offers intensive and structured care for individuals who would require a higher level of care, such as residential or inpatient hospitalization, in the absence of this service. MH-PHP services are short-term, non-residential interventions that are more intensive than outpatient services and that are required to stabilize an individual's psychiatric condition. Partial hospitalization focuses on the overall treatment of the individual and is intended to avert or reduce in-patient hospitalization.

Eligibility Services
Adults aged 18 years and older PHP operates five days per week, five hours every day (11am- 4pm).
Must Have a Mental Health Diagnosis Typical stay in PHP is 4-6 weeks; PHP continuations requested as needed.
Must not be in need of a higher level of care Light morning snacks and lunch are provided daily. Transportation provided as needed.
Must have a MH disorder that results in significant functional impairments in major life activities. PHP provides professional clinical services that include, individual, group and family counseling, skills restoration, medication management, health literacy counseling/psychoeducational activities, peer recovery services, crisis intervention and care coordination.
Ideal transition service for those individuals completing residential treatment or inpatient hospitalization Co-Occurring capable program
Must agree to fully participate in the entirety of the program All Medicaid MCO’s accepted, commercial insurance and Medicare must have prior approval
