Mental Health Skill-Building at Epic Health Partners is a Medicaid-funded program that helps adults with serious mental illness develop the essential skills they need to live safely, independently, and with greater confidence in the community.
This is not traditional therapy — it’s hands-on support that happens in real life, not just in an office. Our team works directly with clients to improve functioning in areas like personal care, medication routines, budgeting, communication, and navigating daily responsibilities.
For many individuals, it’s the difference between surviving day-to-day and building a life worth living.

What’s Included in Mental Health Skill-Building
In-home or community-based 1:1 sessions
Development of individualized goals and skill areas
Support with personal hygiene and health maintenance
Medication management routines
Budgeting, shopping, and meal preparation skills
Transportation training and appointment coordination
Conflict resolution and communication skills
Functional literacy and time management coaching
Coordination with outpatient therapy or psychiatric providers

Who Is This Program For
- Have a qualifying mental health diagnosis (e.g., schizophrenia, bipolar disorder, major depression)
- Are at risk of losing housing, independence, or community placement
- Struggle with day-to-day functioning or basic self-care
- Have a history of hospitalization or need to avoid readmission
- Are already engaged in outpatient therapy or psychiatric services
- Are eligible for Virginia Medicaid and meet medical necessity criteria
