Hospitalization Program (SUD-PHP)
Hospitalization Program (SUD-PHP)
A Partial Hospitalization Program What Can We Expect?
Usually, a professional clinician will conduct a diagnostic interview to assess your medical history before starting a partial hospitalization program (PHP). In addition, you will be screened for drugs and alcohol and assessed for mental health conditions that may require further care. The data will be used by a multidisciplinary team of medical and mental health professionals to create a personalized treatment plan that includes rehabilitation objectives, a discharge strategy, and a relapse prevention plan.
How To Make A Referral For SUD-PHP
At Epic Health Partners, we recognize that breaking bad habits and behaviors can be challenging. We also know, however, that everyone can recover and progress. A partial hospitalization program, when combined with other addiction treatment initiatives, can assist in establishing beneficial coping mechanisms and good behaviors that can break harmful behavioral patterns.
Counseling allows people to address the underlying emotions and mental health issues that may be causing addiction. When used in tandem, these tools can help people get back on track. Get in touch with our team if you or someone you care about is struggling with a substance use disorder and would like our assistance as they reintegrate into society.
